Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to high pressure explosion: At Swallow Unit #2 compressor site an underground low pressure (125 psi) sump tank exploded as a valve from a high pressure line (800 psi) was opened. explosion. 1. Among others, one feasible and acceptable abatement method for this hazard would be to use air receivers as designed or installed in accordance with the ASME Boiler and Pressure Vessel Code. 2. API 54-1999 under pressure equipment section recommends not to operate at pressure greater than the working pressure and to have installed pressure relief devices to discharge at a pressure equal to or less than the rated working pressure. 3. Instruct all employees on the recognition and avoidance of hazards associated with the use of high pressure during purging of low pressure waste oil sump tanks as unsafe condition applicable to his work environment nor to control or eliminate any hazard.
