Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not always furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees are exposed to the condition(s) listed below: On or about 09/19/03, one employee was fatally injured and others were exposed to the following combination of hazards: the hazard of extracting tubing at a high speed; the hazard of the drill bit ramming up into the slips which were chained to the employee work platform; the hazard of having the hinged work platform upset; the hazard of falling into equipment or off the work platform. Among other methods, one feasible and acceptable abatement method to correct the hazards would be to anticipate the surfacing of the drill bit by documenting its depth, ensuring its location is accurately understood, and following accepted work rules of: not chaining the slips to the work platform and slowing the extraction of tubing.
