Text For Citation: 01 Item/Group: 001C Hazard: LOCKOUT
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the following elements, actions and done in sequence as required by 29 CFR 1910.147 (d)(1) (d)(6): a) The hydrostatic test procedures did not require, nor was a lock-out or tag-out device, or any other acceptable means of isolation of energy (preventing unexpected re-introduction of air, or over-pressuring by compressed air or water) used on the tank or air line to the tank undergoing a hydrostatic test procedure on 3/4/05. Air pressure was applied through an air line to the tank to assist in the tank draining. Paragraph (d)(4)(i) requires "Lock-out or tag-out devices shall be affixed to each energy isolating device by authorized employees." b) The hydrostatic or pneumatic test procedures did not address, nor was the stored air pressure fully dissipated from the tank being drained following a hydrostatic test on 3/4/05 prior to an employee opening the tank. Paragraph (d)(5)(i) requires "All potentially hazardous stored or residual energy be relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating device" Abatement Certification and Abatement Documentation required.
