Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the OSH Act of 1970: The employer did not furnish employment or a place of employment which was free of recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were subject to fall hazards: On or about March 20, 2004, employees were exposed to fall hazards of 4-14 feet onto concrete floors, and to the hazard of being struck by falling stock, while removing insulation rolls, while working from wooden "gridracks" (job-made wooden ladders). Among useful and feasible abatement methods would be to remove the job-made ladders from service and until a weekly inspection of all ladders, landings and ladder lashings is developed, implemented, and documented per ANSI A 14.4-1992, sections 7.1 and 7.2. CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.
