Text For Citation: 01 Item/Group: 003 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed the hazard of falling while using a derrickman's escape line: a) Caza Drilling, Rose Ranch 11-12B sec10, 7s,93w, Rifle, CO: On or about 5/01/03, the derrickman's escape line leading from the "monkey board" on Rig 11 was tied to an inadequate anchor point below. Condition exposed the would be user to a fall hazard as the anchorage point was not capable of supporting the anticipated load. Among other feasible and acceptable abatement methods to abate this hazard is to ensure that the ground anchor point is able to withstand a pull of at least 3,000 pounds as found in the American Petroleum Institute, API Recommended Practice 54, 1999 in section 6.10.1 Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
