Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) Caza Drilling, Rose Ranch 11-12B sec10, 7s, 93w, Rifle, CO: On or about 5/01/03, the auxiliary escape line attached to the "monkey board" on Rig 11 was obstructed due to a parked vehicle in the landing area. Condition exposed employee to a hazard of colliding/striking a parked vehicle. Among other feasible and acceptable abatement methods to abate this hazard is to ensure that all escape line routes are kept clear of obstructions as referenced by American Petroleum Institute, API RP 54 1999 in section 6.10.1. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
