Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck/crushed by equipment during oil and gas well drilling operations. a) Caza Drilling, Rose Ranch 11-12B sec10, 7s,93w, Rifle, CO: The employer did not adequately train and instruct each employee on safe work practices including the recognition and avoidance of unsafe conditions associated with assigning new personnel to operate the drilling console on Rig 11. On or about 4/30/2003 an employee was seriously injured after being struck by pipe tongs that were inadvertently actuated by an inexperienced employee operating the drilling console. Among other methods, one feasible and acceptable abatement method to correct this hazard is implement an effective safety program which specifically outlines methods and procedures for all employees whenever inexperienced personnel are assigned to new positions. Observation of new personnel work performance should be maintained until the applicable supervisor is satisfied that the personnel can fill the position in a safe and effective manner as found in the American Petroleum Institute, API RP54 1999, Section 6.
