North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Struck by hazards from the steel rim, locking tabs of the rim and other components of the curing rim assembly in the event of a failure. a)facility, tire re-capping/assembly area - employee periodically inflated a pre-cured, re-treaded, truck tire with curing tube and 2 part locking rim, with an unknown amount of air pressure, which could cause the assembled tube, and 2 part, locking, steel curing rim, to have a catastrophic release of air pressure or explosion. On or about 2/27/03, an explosion of the assembled tire and rim did occur, resulting in fatal injuries to employee. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1. Regulate the amount of air pressure available to the air inflation hose, by use of an in-line air pressure regulator, to a maximum pressure of 30 psi as indicated by the manufacturer, which was stamped on the steel curing rim. 2. Provide an in-line air pressure gage at the air nozzle to indicate the air pressure in the curing tube. 3. Provide non-locking air hose connectors. 4. Provide safe procedures for re-working or breaking down non sealed re-treads.
