Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: On or about October 3, 2002, employee(s) operating the Toshiba VTL #1107 (S/N 410105) were exposed to the hazards of fractures, concussion, crushing injuries, etc. from being struck by thrown parts and /or damaged machine components. One feasible and acceptable method of abatement for this hazard is to incorporate a written, documented risk assessment with written instructions for machining of parts in accordance with ANSI B11.6-2001 Clause 5.1 concerning "Task and Hazard Identification" and Clause 5.2 concerning "Risk assessment/ risk reduction" regarding such tasks factors as clamping methods; table operating speeds; tool feed rates; and part size, configuration, and/or center of gravity.ation,
