Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: At the Willie Brittian #7 oil well site approximately 6 miles northwest of Velma, Oklahoma, on March 6, 2003, during the tubing extraction of the well service operation, one employee was fatally injured and others were exposed to a combination of hazards: the hazard of the tubing anchor ramming up into the slips which were chained up under the employees work platform; the hazard of having the hinged work platform upset, and the hazard of falling into equipment or off the work platform. Among other methods, one feasible and acceptable abatement method to correct the hazards would be to anticipate the surfacing of the tubing anchor by documenting its depth, ensuring its location is accurately understood, and following accepted work rules of: unchaining the slips when nearing the anchor location, and slowing the extraction of tubing when bringing the anchor to surface.
