Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: At the Newcastle oil well service site, west of city in McClain County, December 19, 2002, employees were exposed to the hazard of being struck by a fractured (pull sub) casing pipe freshly welded on to the existing well casing pipe. In extracting the pipe with approximately 80,000 lbs. of tension using the service rig derrick, the pipe failed in the welded area, striking and fatally injuring an employee. Among other methods, one feasible and acceptable abatement method to correct the hazard would be to establish and enforce work rules which would require that all critical welds affecting crew operations be inspected by a competent welding person.
