Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Title 40.1-51.1(a) Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being crushed by a truck crane boom while working in proximity to the crane boom while the power-take-off (PTO) was engaged. (a) August 29, 2001, an employee was killed when he engaged the PTO before connecting the crane pendant control to the Jomac Hydraulic Model 3019 truck mounted articulating crane that he was operating. The crane boom moved, causing the victim to be caught between the crane boom and the truck cab. The Owners/Operators Manual for the Jomac Hydraulic Model 3019 articulating crane made the following disclaimer; WARNING! Do not attempt to service or repair the crane when it is operating or when the PTO is engaged. This warning is repeated in a list of operating rules in the owners/operator manual. Truck crane operators routinely engaged the PTO before connecting the crane pendant control to the truck. One feasible and acceptable method, among others, to correct this hazard is for the employer to train all truck crane operators for the safe use and operation of truck cranes, including the prohibition against engaging the PTO before connecting the crane pendant control to the truck, in accordance with the warnings, rules, and operating procedures found in the manufacturers Owners/Operators Manual.Operators
