Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At ground elevation at Station 271 + 00 of the new bridge over the Cuyahoga River on the Ohio Turnpike on 01/03/02: An existing steel girder approximately 69 feet in length x approximately 7 feet in width was lifted by a Caterpillar Model 330 B excavator in a means not approved by the equipment manufacturer. Among other methods, one feasible and acceptable means to correct the hazardous condition may include: a. Follow the written instructions provided by the Equipment Manufacturer, supplemented by additional instructions from the Equipment Manufacturers Institute Safety Manual for Operators and Mechanics, relative to the use of a lifting bar and lifting points when lifting loads with excavators. b. Follow the written instructions contained in Power Crane and Shovel Association (PCSA) Standard Number 5, paragraph 5.04.5 regarding suspension of loads from the manufacturer's specified load point.
