Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following condition(s): a) Williams Land Rig #1, H&P Rig 136, Amelia, LA - Employees involved in pressure testing operations of a lubricator assembly were exposed to the hazard of being struck by high pressure gas and/or well fluids due to a failure to ensure that the lubricator had a minimum of two (2) bleed valves located on the main body on the lower end of the bottom section and that the two (2) valves be opened when bleeding pressure from the lubricator as specified in API RP 54-99, Section 13, Sub-Section 13.6, paragraph 13.6.5(a). Among other methods, one feasible and acceptable abatement method to correct the hazard would be to ensure that thorough analyses have been conducted and that all necessary procedures have been carried out prior to continuing operations after a pressure test has been conducted.
