Text For Citation: 01 Item/Group: 001 Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of a flash fire and/or explosion in that flammable gas can accumulate at the base of the flare if a flame source is not always present: (a) On or about 06-26-02 at 12:30am a production test operator was lighting a flare at a oil well site near Burton, Texas. He opened the well up and let the gas escape to a flare line. He then went up wind from the flare to light a rag to be thrown into the flare with his cigarette lighter. At that point he was engulfed in flames and burned over 90of his body. Among other feasible and useful abatement methods to correct this hazard, one method would be to always assume to presence of combustible hydrocarbons and have the flame source present before gas is released to eliminate to hazard of flash fires, ensuring all personnel are safely positioned away from flare when opening the well.
