Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards: a) The employer did not furnish a workplace free from recognized hazards in that employees were exposed to hazards of rotating paddles and shafts on the Glycol Chilled Massage sausage blender (s/n 2426) because the tank's gate/guard was not capable of being positively secured in the open position to prevent the accidental closing and machinery startup while employees conducted sanitation work inside the tank. One feasible means of abatement is to install positive locking latch or device that has to be physically released before the gate/guard can be closed. Another abatement means is to install a motor reset button at a location a sufficient distance from the blender's tank to remove the employee from the machinery's danger zone during gate/guard closure and start up.
