Text For Citation: 01 Item/Group: 001 Hazard: UNAPOPPROC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. On September 25, 2001 at 6000 E. Berry St., an employee was hydro-testing a stripper bowl outside at approximately 15,000 psi. The plug weighing approximately 80 pounds blew vertically into the air. An employee working inside of an adjacent building was fatally injured when the plug came down through the roof striking him. Feasible and acceptable method to correct this hazard is: 1. Perform hydro-test in an enclosed testing area. 2. Communicate to all employees that hydro-testing is being conducted by means such as, but not limited to a horn, alarm or PA system.
