Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees of Superior Services of Horicon LLC that operate rear loader trucks were exposed to the hazard of being struck by the container when the container's trunion bar, container bar or container pins were not held in place by the container latches. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: 1. Comply with ANSI Z245.1-1999, American National Standard for Equipment Technology and Operations for Wastes and Recyclable Materials, Mobile Wastes and Recyclable Materials Collection, Transportation, and Compaction Requirement - Safety Requirements: "6.2.5.7.4 Rear loader container handling When operating rear loader equipment with trunnion bar container lifting devices: a) Ensuring that latches that secure the container to the lifting device are engaged and" 2. Comply with ANSI Z245.1-1999, American National Standard for Equipment Technology and Operations for Wastes and Recyclable Materials, Mobile Wastes and Recyclable Materials Collection, Transportation, and Compaction Requirement - Safety Requirements: "6.1.4.1 General Monitoring the employee's activities and taking appropriate action to ensure adherence to safe practices and the employee requirements of this standard;"
