Text For Citation: 01 Item/Group: 001 Hazard: CONFINED
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)The hazards of entering into a permit-required confined space without apppropriate Grade D Breathing air when the atmosphere was determined to be hazardous, without knowledge of proper use and limitations of equipment for control of potential hazards that were likely to be encountered during entry, and a written emergency response plan which included a retrieval system to include harnesses, lifelines and mechanical lifting devices (Full body harnesses should have been donned prior to entrywhere possible retrieval was foreseeable) while working in the #5 casting tank at Olin Brass Mill, Illinois Rt. 3, East Alton, Illinois on July 1, 2003. One feasible means of abatement to correct these hazards includes, but is not limited to, full implementation of the elements required by the American National Standards Institute (ANSI) standard Z117.1-1995 Safety Requirements of Confined Spaces. **ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.**D
