Employer knew or should have known that three maintenance mechanics while removing a blind flange cover on the DR-501 reactor that utilized hydrogen in the Purified Terephthalic Acid unit of Plant Two were exposed to the hazards of fire and explosion form a hazardous atmosphere. A feasible and useful method to correct the hazard, among others is to: 1) Install and maintain a hydrogen detector to alert employees of the presence of hydrogen inside the reactor prior to line/flange breaking. 2) A vent system on all nozzles, man ways or other high points on the DR-501 reactor and visually verify the overflowing of water from all vents.
