Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 2-17-01 employee was stringing Sealtight, electrical conduit, for a vibration switch while the rod string spacing was being adjusted. He was struck in the head by the crank arm and dragged on the ground as the counterweights rotated killing him. One method of abatement among other feasible and useful abatement methods would be to secure the counterweight and lockout/tagout all energy sources before work is done on the unit.
