Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 02-10-01 a flagman was exposed to the hazard to being crushed by a fuel truck backing up in the landfill pit. Among other methods, one feasible and acceptable abatement method to correct this hazard is to : 1) Install an audible backup alarm that will operate automatically when the truck is in reverse, 2) Develop and implement procedures to ensure employees are not in the danger zone where vehicles are maneuvering, 3. Make sure equipment operators are adequarely trained to safely operate the equipment.
