Text For Citation: 01 Item/Group: 001 Hazard: GUARDING
ORS654.010: The employer did not furnish a place of employment which was safe and healthful for employees, and did not do every other thing reasonably necessary to protect the life, safety, and health of the employees: a) The employer recognized there was a problem with the parts being ejected, yet failed to take effective corrective action to prevent reoccurrence of similar accidents or near misses; 1) The employer had knowledge of an alternative method in ensuring that the chuck jaws would hold the part or if the part became dislodged, it would be contained within the jaws; 3) The operators were not provided adequate instruction in properly setting the chuck jaws and review of the operating manual so that they could learn all the intricacies of the lathe operation. OR IN THE ALTERNATIVE OAR 437-001-0760(1)(a) would apply. The employer shall see that workers are properly instructed and supervised in the safe operation of any machinery, tools, equipment, process, or practice which they are authorized to use or apply.
