Title 40.1 - 51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard from fire due to: A.) A flash flame (flashback) shot out of the open 10 inch pipe tuyere of cupola #1, burning two individuals about the face. One feasible and acceptable abatement method, among others, to correct this would have been to light the cupola in accordance with established procedures through the sight glass hole.
