Text For Citation: 01 Item/Group: 002 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Heath Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm of employees in that employees were exposed to a electrocution hazard: a. Second Floor, Near Concrete Pillar 2D11 - An employee was standing inside an elevated lift cage that was hoisted on the forks of a Yale RF-1071 electric powered forklift truck. This employee used an approximately 9 foot, 11 inch cylindrical metal pipe to push the vertical control lever on this forklift truck. The lever was in the position that raised the lift cage. The employee was alone. The lift cage came into contact with an overhead fluorescent lamp fixture that was suspended from the ceiling. This employee was electrocuted. Among other methods, feasible and acceptable abatement methods to correct these hazards would include: 1. Maintain a trained operator in a position to control the forklift truck at all times when the platform is occupied by personnel as stipulated by American National Standard, "Safety Standard for Low Lift and High Lift Trucks", ASME B56.1-1993, paragraph 4.17.3 (n). NOTE: ABATEMENT DOCUMENTATION IS REQUIRED.
