Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the conditions listed below: Greywolf Drilling Rig #91, 6 mi NW of Kaplan, LA, NE of Dogwood Rd. On or about 10 Mar 98, the following conditions existed on Rig #91: a) Each end of the 2.0" hose that was used as the rotary hose (chiksan line) was not secured by a cable or chain in accoradnce with API RP54 Para 8.7.7 which states: "The pump end of the rotary hose should be securely fastened to the derrick or mast by a cable or by a chain clamped to the hose and to the derrick or mast leg. The swivel end of the hose should be secured by a similar cable or chain, with the other end of the cable or chain affixed to the swivel." Safety chains were not available on the rig. b) The 2.0" hose that was used as the rotary hose (chiksan line) was used in a manner in which it was not intended to be. The rotary hose is usually attached from the top of the standpipe to the swivel. At this location, one end of the rotary hose was attached to a valve located on the standpipe manifold and the other was attached directly to the drill string. These conditions exposed the employees to the hazards and injuries related to the sudden and unexpected failure of a hose fitting and the release of pressurized drilling fluids. Among other methods, one feasible and acceptable abatement method to correct the hazard is: a) Assure that safety chains are attached to the hose prior to rig-up and develop procedures to assure that the hose is secured to the rig structure prior to use. b) Assure that the rig is properly equiped with all the necessary equipment prior to rig-up and that any and all missing equipment is replaced promptly.
