Text For Citation: 01 Item/Group: 002 Hazard: UNAPEQUIP
Title 21 V.S.A. Chapter 3, Subchapter 5, Sec. 223(a) of the Vermont Occupational Safety and Health Act. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, the wire ropes and cables on the Franki pile driver were not inspected and maintained in accordance with Wire Rope Users Manual. Location: Rutland Transit Center Violation: The 5/8ths inch diameter 6X25 IWRC, right regular lay, bright finish wire rope used to raise and lower the trevie, was not maintained in accordance with the Wire Rope Users Manual dated 1993 in that the cable was allowed to be in use past its safe life span resulting in a catastrophic failure causing serious permanent injury to an employee. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to ensure that all cables and wire ropes were inspected for wear and tear, broken wires and corrosion as recommended by the Wire Rope Users Manual.nded
