Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falling from or being struck or crushed by hatch covers during movement: (a) On or about August 20, 1996, employee(s) were exposed to hazards of falling from, or being crushed by, hatch covers while sitting and/or standing on covers during the movement of adjacent covers. (b) Employees were exposed to falling from, or being struck by, hatch covers while staying on top of hatch covers while adjacent covers are being "choked". Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement, monitor and enforce work rules to ensure that employees are not on the top of hatch covers and are in a safe location prior to allowing hatch covers to be moved.
