Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Struck-by hazards, in that employee(s) performing "reverse pull" techniques during insertion of a concrete pipe into a corrugated steel sleeve was exposed to the hazard of being struck by the pull wire. Among other methods, feasible forms of abatement for this hazard include but are not limited to: (1) Obtain an approved engineering plan for the pipe insertion process prior to attempting the insertion; (2) Utilize an insertion method which permits employee(s) to be positioned outside tool range during the insertion process, and; (3)Inspect all portions of the tool assembly for damage or signs of fatigue prior to initiating each stage of the insertion.
