Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns, electrical shock, and smoke inhalation from fires, electrical contact, and explosion due to the use of unapproved electrical equipment: In the following instances electrical equipment was not operated or maintained in a safe condition: a)Barn 2 - The electrical conduit for the auger drive motor was broken. b)Barn 71 and 72 Aisle - The constant fan control switch contained 230 volts, and a red conductor was spliced to a white conductor without the proper identification. The constant fan control switch was rated for 115 volts. c)Poland Incinerator Building - The #1 fuse contact was outfitted with a screw which bypassed the over-current protection or fuse. d)Poland Barn 1 - The pump switch was not provided with strain relief. e)Poland Barn 1 - The conductors for the two feed motors at the barn entrance were not provided with strain relief. f)Montville Boiler Room - The black outlet that was rated for 15 amps was connected to a 30 amp circuit. The outlet was not provided with a ground wire. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install approved gaskets, covers, hangers, conductors, switches, and fittings. Your company was previously cited for a violation of this Occupational Safety and Health Standard which was contained in OSHA Inspection Number 300298718, Citation Number 2, Item number 1, issued on May 28, 1999.d
