Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to internal injuries, fractures, or death from possible collapse of elevated catwalks, roofs, and structures: In the following instances structures were not adequately supported or guyed: a)Feed Mill - Most of the wire-rope clamps on the guy wires of the catwalk towers were installed improperly and in insufficient numbers. b)Feed Mill - The guy wire splice adjacent to the truck bay contained wire-rope clamps that were not designed for joining wire-ropes. c)Feed Mill - The anchor adjacent to the truck bay contained cracked concrete. d)Feed Mill - The guy wires throughout the facility were not properly tensioned or torqued. e)Poland Incinerator Building - The roof rafter adjacent to the smoke pipe was defective due to fire and heat exposure. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install the proper hardware and framing members. Your company was previously cited for a violation of this Occupational Safety and Health Standard which was contained in OSHA Inspection Number 300298718, Citation Number 1, Item Number 2, issued on May 28, 1999.d
