Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard : Employee was exposed to the hazard of being struck by a falling pipe due to not having a safety latch or other positive locking method: a.A 5-1/2 inch side door elevator was used with chain and hook, but with no safety latch to prevent the elevators from opening up. b.The side door elevator was not used with an other positive locking method. This occurred on or about April 8, 1997 on Tetra Technologies, Inc. Workover Barge Rig #460. Among other methods, one feasible and acceptable abatement method to correct this hazard would be installation of a safety latch on the hook or an other positive locking method, such as specified in Section 8.8.3 of American Institute (API) RP 54, essential elements of this procedure would include: 1.Installing a safety latch on hook or other equivalent device to prevent accidental release of the load being hoisted or lowered. 2.Use an elevator that is equipped with a positive locking method.
