Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from overturning forklifts due to the fact that operator restraint systems (seat belts) provided on forklift trucks were not required to be used, as required by ASME B56.1-1993: (a) Jobsite: On or about 08/03/98, an employee operating a forklift truck was not wearing the seat belt provided and was fatally injured when the forklift overturned. Abatement Note: One feasible and acceptable abatement method to correct this hazard is to require and effectively enforce the use of operator restraint systems while operating forklift trucks provided with them.
