29 CFR 1910.176(b): Storage of material created a hazard. Bags, containers, bundles, etc. stored in tiers were not stacked, blocked, interlocked, and limited in height so that they were stable and secured against sliding or collapse. (a) The Andersons, 521 Illinois Avenue, Maumee, Ohio: On or about March 6, 2012, the employer did not ensure that tiered super sacks filled with Carrier Blend and other process materials did not create a hazard when stored. The lower sack of a tier was punctured releasing the contents and causing the stack to become unstable. The upper super sack fell onto an employee who was attempting to stop the release resulting in severe injury to the employee.
