29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes (except for "replacements in kind") to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process. On or about February 22, 2012 and at times thereafter, the employer did not implement a management of change (MOC) procedure for the removal of a flare drum pump in the crude unit and subsequent use of a vacuum truck suction procedure to remove flammable liquids. This condition exposed employees and contractors to a flash fire/explosion and chemical release hazard.
