29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Employees cleaned, set up, and performed maintenance on the two vertical injection molding machines, without first isolating the energy sources, such as the electrical disconnect and the pneumatic ball valves and blocking the gravitational energy of the ram. (b) Employees cleaned, changed tags on the dies, and set up the mechanical power presses, without first isolating the energy sources, such as the electrical disconnect and the pneumatic ball valve and blocking the gravitational energy of the ram. (c) Employees changed the blades on the Powermatic Vertical Saw without first isolating the electrical energy source, such as the electrical disconnect or the plug.
