Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to rotating parts and an inrunning nip point hazard: a) A position mean to lock out electrical power for the Gessner Hi- Torque nappers, company nos. 5512 and 5511, was not provided. Employees were exposed to inadvertent operation of the napper rollers, tension roller and braker rollers during the threading sequence, September 21, 1989. AMONG OTHE METHODS, A FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: 1) To provide zero mechanical state (ZMS) as to provide maximal protection against unexpected mechanical movement of equipment by locking out the electrical and mechanical power to the equipment. In addition, a written program for ZMS should be established with training of all affected personnel.
