Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working at Pier 6, Port of Providence, were exposed to serious injuries while work with jobsite-fabricated jack stands that were not adequately designed and welded. The jack stands, which were fabricated on site from 6"x8" I- beams stock, were not cut square and therefore provided unlevel base support for the jacks. In addition, 1/2" steel plates were welded to I-beam stock improperly, in that they were only tack welded, instead of fully welded, and were welded on only one side, instead of on both sides. One feasible and acceptable abatement method to correct this hazard is to use either properly designed, cut, and welded jack stands, or jack stands of a solid construction, or manufactured cylinder bases to provide support for the jacks.
