Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flying metal fragments when the Bock Model 755 centrifugal extractor failed after it was loaded and operatored well in excess of its rated load capacity in the Silver Cyanide Manufacturing Area on or about October 29, 1987. Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop and implement a Standard Operating Procedure (SOP) for all centifugal extractor operations which would include, but not limited to: 1) Posting of the maximum rated load capacity and speed at each extractor. 2) Instructing and training extractor operators in the recognition and avoidance of unsafe conditions associated with extractor operations and how to control or eliminate such hazards. 3) Establishing a periodic inspection and maintenance program to check and repair safety devices and the structural/mechanical integrity of all centrifugal extractors on a regular scheduled basis.
