Text For Citation: 01 Item/Group: 001 Hazard: DUST&FUMES
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fire/explosion of wood dust: a. Dust Collection System - The infrared sensors for the Flamex system were not maintained as required, potentially allowing dust and debris to build up over the sensors and test eyes, therefore decreasing their sensitivity. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to design, install, and maintain the system as per the manufacturer's recommendations. b. Dust Collection System - The Fenwal Delta-Pac fire suppression system was not properly inspected, tested and results documented in that, for example, the factory-charged containers were not checked for loss, the enclosure was not inspected and persons assigned to do the inspections were not thoroughly trained. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to design, install, and maintain the system as per NFPA 12A - Halon 1301 Fire Extinguishing Systems. c. Dust Collection System - The Fenwal X-Pac explosion isolation system was not properly inspected, tested and results documented in that, for example, the factory-charged containers were not checked for loss, the enclosure was not inspected and persons assigned to do the inspections were not thoroughly trained. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to design, install, and maintain the system as per NFPA 12A - Halon 1301 Fire Extinguishing Systems. d. Dust Collection System - The explosion doors on the Moldow Dust Collector were not properly torqued to the manufacturer's specifications of 3.5 - 4.3 ft-lbs. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain the system as per the manufacturer's recommendations.
