29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling. a) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap in the manager's office. See IMG 1117, the white relocatable power tap on the left side of the image is serially connected to the black relocatable power tap located in the lower, middle portion of the image, on or about March 21, 2024. b) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap in the manager's office. The white relocatable power tap on the right side of the image is serially connected to the relocatable power tap located in the lower, middle portion of the image, on or about March 21, 2024. c) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap in the manager's office. The green flexible power cord is serially connected to the white relocatable power tap that terminates into a wall receptacle, on or about March 21, 2024. h) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap in the employee's breakroom. The white relocatable power tap is serially connected to the black relocatable power tap, on or about March 21, 2024. j) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap on the right wall, behind the primary retail counter. The white relocatable power tap is plugged into the relocatable power tap that is secured to the vertical supports of the adjustable shelving, on or about March 21, 2024. k) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap behind the primary retail counter's, bumped out workspace. Two white relocatable power taps are serially connected to one another and terminate into a receptacle, on or about March 21, 2024. l) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap behind the primary retail counter's, bumped out workspace. The black 5-way relocatable power tap is connected to a flexible power cord and terminates into a receptacle, on or about March 21, 2024. m) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to another relocatable power tap located behind the TV display table near the front, center of the customer retail area. The white Belkin relocatable power tap is serially connected to the pink and orange relocatable power tap which terminates into the receptacle suspended from the ceiling, on or about March 21, 2024. n) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due to operating relocatable power taps that were serially connected to flexible power cord located above the outdoor tool display wall on the left wall of the customer retail area. The free floating, white relocatable power tap is connected to the black flexible power cord (supported on the display hooks) which terminates above the display cabinet, on or about March 21, 2024. o) 510 Kenhorst Plaza, READING, PA 19607 - Employees were exposed to electrical hazards while conducting retail operations due a metallic junction box that was not mounted to a surface to ensure employees are not manually interacting with the receptacle box. Metal junction boxes must be mounted in accordance with Underwriters Labatories Standard 5.14A. A non-contact tester confirmed the present of voltage while held to the black flexible cord traveling from the junction box, on or about March 21, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
