29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) 223 Park Avenue, Manalapan, NJ 07726: Employees who operate a Komatsu Forklift, model FG15ST-17 serial Number 6070210A were not trained and evaluated by the employer to verify that they had completed training and evaluation in the operation of the forklift to verify they were competent to operate the forklift on or about 12/30/2023. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
