Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to struck-by hazards: (a) On December 22, 2023, at or near 63rd Street NW and 131st Avenue NW at the Jennifer 26-35 #2TFH wellsite near Epping, North Dakota, an employee was exposed to struck-by hazards while conducting well servicing operations where well control practices were not followed resulting in the pressurized release of gases from the well. Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to: 1. Follow the American Petroleum Institute Recommended Practice 54, Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations section 6.1 General (Operations and Procedures); Subsection 6.1.1. Well Control shall be maintained as needed for the type of operation 2. Follow the American Petroleum Institute Recommended Practice 59 - Recommended Practice for Well Control Operations, Section 11.11.1 Barriers to Flow
