29 CFR 1915.155(a)(1): The employer did not ensure that affected employees wore protective helmets when at risk from being injured by falling objects. On or about November 11, 2023, inside the Mo-Gas Tank 4C 9-182-1 of the USNS 1st Lt Jack Lummus, employees were not wearing hard hats while other employees were working above them with tools such as hoses, knives, and flashlights, exposing the employees below to an overhead hazard.
