29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships: The employer was failing to protect employees from explosion hazards. This was most recently documented at a worksite, at or near, 3305 West Old Potash Highway, Alda, Nebraska on October 13, 2023. The process hazard analysis (PHA) did not list as a safeguard the operating limits to include the maximum number of personnel in the operating building nor were there any postings specifying the maximum number of personnel in the building.
