Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees engaged in loading/unloading of mail utilizing electric pallet jacks such as Toyota, Model 8HBE40 to and from the dock and inside of the trailer while motor vehicles connected to the trailers maneuvered in forward gear were exposed to crushing, struck by and falls hazards from the dock measuring approximately 4 feet 1 inches to the ground.: a) Jobsite: 7500 Lindbergh Blvd. Philadelphia, PA - Loading docks: On or about August 21, 2023, The employer did not have an effective system to notify truck drivers when they can pull away from the loading dock once the employees loading/unloading mail from an over-the-road tractor/trailer have ceased, exposing employees to crushing, struck-by and falls from the loading dock to the ground when the motor vehicle connected to the trailer pulls away from the loading dock while the employee on the electric pallet jack is inside the trailer or in the process of moving from the trailer to dock.
