29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not deenergized before the employee worked on or was near to them: Location: Annex Building -mechanical/boiler room a. Maintenance employees worked on energized 20amp, 110Volts up to 30amp, 220 volts circuits, conducting electrical work such as, but not limited to, changing light fixtures, GFCI Outlets, unit blower motors, and changing boiler circulating pump contactors. The employer did not ensure that employees de-energized or performed lockout/tagout for any circuits brakers prior to disconnecting electrical components, exposing employees to electrocution hazards. Violation occurred on or about August 14, 2023
