29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: a) Excavation located on Massee Ln.: On or before August 10, 2023, the employer deviated from the specifications, recommendations, and limitations determined by the manufacturer of the shoring used to protect employees performing sewer line installation while working inside a 21-feet deep trench excavation and did not request written approval from the manufacturer exposing employees to engulfment and struck by hazards.
