29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) In the warehouse: On or about August 02, 2023, employees were exposed to struck-by and crushed-by hazards in that the Yale forklift was outfitted with a homemade boom extension that did not have written approval from the manufacturer of the forklift to be used on the forklift.
