29 CFR 1910.147(d)(4): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about July 24, 2023, employees of a maintenance shop were exposed to a struck-by and crush -by hazards at a worksite located at 1900 E Blaine St, Springfield MO. Employees were allowed to work in and around a bus that was being repaired without isolating and locking out all sources of energy including but not limited to pneumatic, mechanical and electrical energy.
